Your doctor may not necessarily know about the care preferences of different patients, so you and your family should start direct talks with your doctor to help make care decisions clearer and easier.
• Ask what needs or symptoms common to your injury or illness would qualify you for home health care and whether you currently fit the qualifying criteria.
• Clarify treatment goals that are important to you, whether they be maximizing the possibility of cure, management of pain and discomfort, or staying home and out of the hospital as much as possible.
• Discuss the practical implications of choosing Medicare-covered home health care with a social worker from a senior center, hospital, home health agency or hospice. Social workers tend to have specific knowledge about the practical arrangements of different care options.
